Let me explain casual cruelty.

Your dad has dementia. He has an infection that is painful. He’s very annoying. Rude. Can’t communicate needs. Incontinent at times.

As a nurse I I know he’s in pain and can’t communicate that outside of being an asshole. I know he cant swallow well. I get an order to crush pain meds and feed them to him in applesauce between him calling me names. I use the appropriate nonverbal pain scale for dementia to chart his signs of pain that aren’t him saying “please give me tylenol”. I laugh and change the subject to the show on tv when he says mean things. He calms down. He even sleeps a few hours. I see him waking up at 6:00 am. He is restless. I give him a dose of tylenol before the oncoming shift. I say to the next nurse hey listen he did really well with medications if he gets agitated this is what worked for me. He’s hurting.

A nurse who does “nothing wrong”: sees order for as needed every six hour pain meds. It’s not scheduled it’s “as needed” which means up to her assessment. She thinks he’s an asshole, gets a sitter to help keep him in bed, charts that he is in 0/10 verbalized pain, charts he was monitored for safety all night, tells oncoming shift he is an asshole, goes home.

That is the difference of 12 hours of pain for your loved one. Both those people are paid the same and both think they do a good job.
